PACE

The Program of All-Inclusive Care for the Elderly (PACE) is a capitated benefit that features a comprehensive service delivery system and integrated Medicare and Medicare financing for frail, elderly individuals that meet Long Term Care level of care criteria.

NOTE: The area served by the Center for Senior Independence in Wayne County is limited to zip codes 48201-48219; 48221-48230; and 48234, 48235, 48236, 48238, 48239, and 48240. The area served by LifeCircles in Muskegon County includes the folliwing zip codes in Ottawa County only: 49403, 49404, 49409, 49417, 49448, and 49456.
