Care Management

Care management is a coordinated care function that incorporates case finding, assessment, care planning, negotiation, care plan implementation, monitoring, and advocacy to assist clients and their families with complex needs in obtaining appropriate services. This program locates, mobilizes and manages a variety of home care and other services needed by a frail elderly person at risk of nursing home placement. An assessment is performed to identify needs, and appropriate services are secured to enable the individual to remain at home. Case managers assist in gaining access to needed services for which a person is eligible. In addition, they monitor the provision of these services as well as initiate and oversee the assessment and reassessment of an individual's level and plan of care.
