Case Coordination & Support Similar to care management, case coordination organizes access to services for persons 60 and older according to their level of need. It includes a comprehensive assessment and subsequent reassessments of persons aged 60 and over with the subsequent development and monitoring of service plans tailored to the individual and their needs. Case coordinators broker existing community services while working to enhance informal support systems when feasible. In addition, case coordinators identify and communicate appropriate community agencies to arrange for services and evaluate the effectiveness and benefit of services provided
