Nutrition Counseling

Nutrition Counseling provides individualized advice and guidance to persons who are at nutritional risk (because of health and/or medication use or chronic illness) about options and methods for improving their nutritional status. An assessment of need is first performed. The assessment will determine at minimum a person's nutritional history, both chronic and acute health problems, and a listing of all prescription and over the counter medications taken. The counseling service must be performed by a registered dietician. The counselor will develop a nutritional plan of care. Nutrition counseling can be provided ion a variety of settings, including a person's residence.
