Outreach Outreach involves agency and programmatic efforts to identify and contact isolated older persons and/or older persons in the greatest social and economic need who may have services needs as well as assist them in gaining access to appropriate services. Outreach does not include comprehensive assessment of need, development of a service plan or arranging for service provision.
