Health Screening Health screening systematically assesses an older individual’s health status under the supervision of a registered nurse in order to identify and/or monitor actual and potential health problems and to determine if referral for medical intervention is indicated. The screen collects basic demographic information about the client along with a health history and nurse assessment. The nurse assessment covers a person’s physical status, mental and social status, vision, hearing, dental health as well as any abnormal signs or symptoms that present themselves. The service also includes the provision of basic health information in response to screening results as well as referrals for medical intervention and follow-up. Looking for Providers?
