Caregiver Case Management This service combines OSA Operating Standard service definitions for Care Management, and Case Coordination and Support for Case Management services provided to a caregiver. The provision of a comprehensive assessment of persons with a complementing role of brokering existing community services and enhancing informal support systems. In addition, a comprehensive assessment, care plan development, periodic reassessment, ongoing coordination and management of in-home and other supportive services to persons aged 60 and over who are in need of a nursing facility level of care due to the presence of functional limitations.
