Long Term Care

Long term care is often needed when a person can no longer perform everyday tasks for themselves. It consists of a range of medical and/or social services designed to help people who have disabilities or chronic care needs. For years the phrase "long term care" was used almost exclusively when referring to nursing home care. Today, it's defined more broadly to include services provided over an extended period of time in a variety of settings including your home, other sites in your community, managed residential settings, and institutional settings.
